needing evaluation or repair, please fill in the appropriate
information. An RMA number will be assigned and you will
be notified via phone or fax.
A representative will
call or email you within 24 hours of submitting this form.

- Please fill out the RMA Request Form below
- Do not ship defective product until you
receive an RMA number from Telios Tech
(Refer all calls to the RMA department - tel: 949.831.847)
Returns Policy
- All returns require an RMA number, which
must be prominently displayed on the box.
- Telios Tech will issue an RMA number within
2 business days after receipt of the completed RMA form.
- Telios Tech must receive the product within
ten days of the issuance of the RMA number or the RMA
number will expire, which may void the warranty.
- All RMA items must be returned in their
original packaging. If you do not have the original packaging,
please notify the RMA department when requesting an RMA
number. We can provide packaging material to you. A fee
may apply.
- Telios Tech will not accept COD returns
or 3rd party returns.
- Warranties are void if manufacturer serial
numbers have been removed.
- The warranty is voided upon the
invoice becoming past due.
